Location: North Highlands, CA

Overview:
The Patient Care Coordinator is tasked with managing both emergency and non-emergency patient transfer requests, utilizing air or ground ambulance services. This role is pivotal in ensuring seamless communication and coordination among various stakeholders including hospital staff, physicians, and transport vendors.

Key Responsibilities:
1. Respond to emergency and non-emergency transport requests via telephone, ensuring a high level of customer service.
2. Utilize discretion and independent judgment to address diverse requests effectively.
3. Gather essential information to facilitate patient transfer and arrange appropriate transportation.
4. Adhere to established policies and procedures related to patient referral and transport coordination.
5. Acquire knowledge of local EMS protocols and regulations, including EMTALA and Title regulations.
6. Collaborate with physicians, case managers, and facility personnel to document transfer requests accurately.
7. Monitor the status and location of ground and air ambulances in relation to patient transfers.
8. Maintain familiarity with the geography of the service area and medical facilities.
9. Prioritize tasks, meet deadlines, and provide updates on ongoing projects.

Documentation:
Complete all necessary documentation with accuracy, ensuring grammar and spelling are checked.

Qualifications:
Experience:
- Preferred 6 months to 1 year in a healthcare setting.
- Customer service experience of at least 1 year is preferred.
- EMS dispatch experience is a plus.
- Knowledge of medical terminology is required.

Education:
- High School diploma or equivalent is required.
- A two-year degree in a related field is preferred.

Skills:
- Current or previously held certifications such as EMT, Medical Assistant, or Case Manager are preferred.
- Must pass all assessments associated with the internal training program.
- Typing skills of at least 35 words per minute are required.
- Proficiency in Microsoft Office and other PC-based applications is essential.

Interpersonal Skills:
- Ability to maintain professionalism in high-pressure situations.
- Capable of working independently and as part of a cohesive team.
- Strong oral and written communication skills are necessary.
- Effective problem-solving and conflict resolution abilities are required.

Training:
All new hires must complete a scheduled training academy and pass a formal skills assessment to continue their training and duties as a Patient Care Coordinator.
