Care Coordinator for Home Health Services

We are seeking a skilled Home Health Physical Therapist to join our team at AccentCare.
About the Role:This is an ongoing opportunity to provide high-quality physical therapy services to patients in their homes, covering areas such as Minneapolis, Maple Grove, and Golden Valley. As a key member of our team, you will be responsible for delivering exceptional patient care, working closely with healthcare professionals to develop personalized treatment plans, and ensuring seamless communication among all parties involved.
Compensation and Benefits:We offer a competitive salary range of $115,000 - $125,000 per year, paid on a per-visit basis. Our benefits package includes medical, dental, and vision coverage, paid time off, professional development opportunities, company-matching 401(k), flexible spending accounts, and more.
Requirements:- A minimum of nine months' experience as a licensed physical therapist
- Preferred community/home health experience
- A Master's degree in physical therapy from a CAPTE-accredited program
- Current licensure to practice as a physical therapist within the state of operation
- Valid CPR certification and a reliable means of transportation
