Your Key Responsibilities
  • Collaborate with our care team to provide ongoing support and communication to patients with chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD)
  • Act as a single point-of-contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up
  • Ensure seamless continuum of care for patients by coordinating with clinical resources and providers
  • Provide patients with education materials and send communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments
