Job Description:- Provide comprehensive nursing care to clients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that meet the unique needs and goals of each client.
- Collaborate with clients, families, and healthcare teams to ensure seamless care coordination and communication.
- Conduct thorough assessments of clients' physical, emotional, and social needs, and develop plans to address these needs.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Monitor clients' conditions and report any changes to the Clinical or Client Services Manager.
- Document all interactions, assessments, and care provided in a timely and accurate manner.
- A current license as a Registered Nurse in the state of Pennsylvania.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare teams.
- Solid computer skills and prior experience with electronic medical records (EMR) is preferred.
