Job Summary:The Registered Nurse is a licensed healthcare professional with advanced medical knowledge who administers skilled nursing care to patients requiring intermittent visits in their homes. She/he provides complex care to the patient and coordinates necessary support services. These nursing services are provided to patients in accordance with a physician's plan of care.
Responsibilities:- Administer skilled nursing care to patients in their homes

- Utilize case manager approach in assessing, implementing, coordinating, and evaluating client care
- Registered professional nurse with a current valid Pennsylvania license
- Minimum of two years of current experience as a professional nurse in a medical/surgical work area and one year of home health experience (preferred)
- Skilled in critical thinking, communication, negotiation, and collaboration
