About the Opportunity

Oakland Family Services is seeking a skilled Wraparound Facilitator to join our Specialized Services for Youth department. As a key member of our team, you will play a vital role in providing facilitation and coordination of the Wraparound model for families enrolled in our program.

Key Responsibilities
  • Facilitate and organize Child and Family Team meetings, ensuring adherence to the Wraparound model.
  • Develop and implement Person-Centered Plans, incorporating strengths assessments, needs assessments, and crisis support plans.
  • Collaborate with the Child and Family Team to identify and engage formal and informal supports.
  • Provide education and support to families on topics such as communication and child development.
  • Advocate for youth and families, accompanying them to appointments and connecting them with community resources.
  • Develop and maintain relationships with community partners, stakeholders, and service providers.
  • Participate in case coordination and case consultations with the treatment team and other partners.
  • Ensure strong fiduciary oversight of flexible funds and present plans to the Wraparound Community Team.
  • Participate in professional development activities, including regular supervision and licensure requirements.
Requirements
  • Bachelor's degree in social work or a related field with state licensure.
  • Minimum three years of experience working with children and families.
  • Significant understanding of clinical and non-clinical systems in Oakland County.
  • Ability to develop and maintain relationships with community partners and stakeholders.
  • Strong communication and advocacy skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
