Key Responsibilities:
- Perform initial and ongoing assessments of clients' needs, identifying areas for intervention and support
- Develop and update individualized care plans, incorporating clients' preferences and goals
- Provide intensive case management services, including accompaniment to appointments and assistance with housing
- Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination
Qualifications:
- Bachelor's degree in Social Science field, Public Health, or closely related field
- 1 year of experience providing case management services to high-risk, medically underserved populations
- Strong communication and interpersonal skills, with ability to build rapport with clients and colleagues
- Proficiency in electronic medical record documentation and basic computer applications
