Job Summary

The Occupational Therapist - Post Acute Care Specialist provides direct rehabilitation care to patients in occupational therapy, as referred by physicians, in accordance with all applicable laws, regulations, and Life Care standards.

Key Responsibilities
  • Develop and Implement Treatment Plans: Create and execute occupational treatment plans for patients, focusing on activities of daily living.
  • Lead Interdisciplinary Teams: Oversee and evaluate care given by occupational therapy assistants, rehab aides, and students.
  • Chart and Document Patient Progress: Accurately and timely chart patient information and progress.
  • Utilize Therapy Software: Effectively use therapy software to document patient information and track progress.
  • Provide Excellent Customer Service: Demonstrate a positive attitude and provide excellent customer service to patients.
  • Participate in Emergency Procedures: Assist in the evacuation of patients during emergencies.
  • Maintain Professional Relationships: Develop and maintain professional working relationships with colleagues, vendors, and other stakeholders.
  • Ensure Confidentiality and Integrity: Maintain confidentiality of proprietary and confidential information and adhere to Life Care's Code of Conduct.
Requirements
  • Certifications and Licenses: NBCOT certified upon hire, with optional renewal; currently registered/licensed in applicable State.
  • Education and Experience: Graduate of an accredited program in occupational therapy (BSOT or MSOT); one year experience in post acute care or related setting preferred.
  • Skills and Abilities: Proficient in Microsoft Word, Excel, and email; knowledgeable of practices and procedures governing functions in post acute care facilities.
