Key Responsibilities- Provide direct patient care, including administering medications, monitoring vital signs, and developing and implementing individualized care plans.
- Conduct thorough assessments of residents' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with residents, families, and healthcare teams to ensure informed decision-making and optimal care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance resident care.
- Active KY Registered Nurse License
- 2-3 years' experience as a licensed nurse in areas including but not limited to Skilled Nursing Facility, Dementia Care, Short Term Rehabilitation, or Orthopedics
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
- Ability to work a varied schedule, including weekends, evenings, and overnights
