Job Summary:

The Speech Therapist will be responsible for providing Speech Therapy services to patients in their homes, as prescribed by their physician. This will involve assessments, administering treatments and therapies, care planning, communication with other healthcare team members, and coordination of care.

Key Responsibilities:
  • Provide Speech Therapy services to patients in their homes, as prescribed by their physician.
  • Conduct assessments and develop care plans to meet the individual needs of each patient.
  • Administer prescribed treatments and therapies to patients.
  • Communicate effectively with other healthcare team members, including physicians, nurses, and family members.
  • Coordinate care with other healthcare professionals to ensure seamless delivery of services.
  • Maintain accurate and detailed records of patient progress and care plans.
Requirements:
  • Master's degree in Speech-Language Pathology from an ASHA-accredited college/university.
  • Current and unrestricted SLP license to practice in the state of practice.
  • Current CPR certification.
Preferred Qualifications:
  • One (1) year of recent experience as a Speech Language Pathologist in an acute care setting.
