Position Overview

The Clinical Nurse Specialist plays a vital role within the patient-centered medical home team. Qualifications and Experience

  • Diploma in Nursing or an Associate's Degree in Nursing
  • Current licensure by the State Board of Nursing
  • BLS certification required
  • Preferred: Two years of experience in a medical office setting

Supervisory Role

  • This position does not have direct supervisory responsibilities.

Key Responsibilities

The Clinical Nurse Specialist will engage with patients, medical providers, clinical staff, and non-clinical personnel to ensure high-quality service delivery:
  • Evaluate patient needs through comprehensive data collection, including history, observation, physical examinations, and diagnostic analysis.
  • Deliver direct patient care, including triage, assessments, procedures, treatments, patient education, and follow-up.
  • Assist patients and providers in securing necessary medicinal prior authorizations.
  • Ensure patients understand the purpose of their visits.
  • Provide preventive care information tailored to patients' age, gender, and health status.
  • Maintain knowledge of practical nursing principles to deliver effective patient care.
  • Familiarity with examination, diagnostic, and treatment room protocols.
  • Ability to interpret and apply clinical guidelines and procedures.
  • Basic knowledge of critical care and chronic illness management.
  • Participate in ongoing training and professional development opportunities.
  • Contribute to office efficiency and patient encounter volumes.
  • Administer vaccines and immunizations to both pediatric and adult patients.
  • Manage daily clinical workflows, including referral management, telephone encounters, lab results, medication refills, and compliance with medication logs.
  • Demonstrate a willingness to learn about community health resources and organizations.
  • Assist in onboarding and training new nursing staff in standard operational procedures.
  • Perform other duties as assigned.

Physical Demands

Hearing: Adequate to perform job duties in person and over the phone. Speaking: Must communicate clearly with patients in person and via telephone. Vision: Sufficient visual acuity to perform job functions, including reading printed materials and computer screens. Other: Occasional lifting and carrying of items weighing up to 10 pounds unassisted in daily activities and up to 30 pounds for specific tasks. Frequent bending, reaching, and repetitive hand movements are required, along with standing, walking, squatting, and sitting throughout the work shift.
