Key Responsibilities- Provide high-quality, patient-centered care to a panel of up to 450 patients
- Act as a health coach, helping patients set and achieve their health goals
- Collaborate with other healthcare professionals to ensure seamless care coordination
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Current State Medical License and DEA number
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty (preferred)
