Responsibilities:

  • Coordinate care for assigned patients
  • Collaborate with healthcare providers to ensure high-quality patient outcomes
  • Develop and implement individualized care plans
  • Monitor patient progress and adjust care plans as needed
  • Provide education and support to patients and their families
  • Communicate effectively with interdisciplinary teams to ensure seamless care coordination

Requirements:

  • Graduation from an accredited school of nursing
  • Current Alaska Registered Nurse License upon hire
  • 2 years of professional nursing experience

Preferred Qualifications:

  • Bachelor's Degree in Nursing
  • 2 years of case management experience in a clinic or outpatient setting
