Job Summary:
The RN Case Manager will coordinate care for a panel of patients, collaborating with healthcare providers and ensuring high-quality patient outcomes.
As a key member of our care team, you will work closely with patients, families, physicians, medical assistants, social workers, counselors, and other clinic support staff to provide comprehensive care.
Responsibilities:
- Coordinate care for assigned patients
- Collaborate with healthcare providers to ensure high-quality patient outcomes
- Develop and implement individualized care plans
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families
- Communicate effectively with interdisciplinary teams to ensure seamless care coordination
Requirements:
- Graduation from an accredited school of nursing
- Current Alaska Registered Nurse License upon hire
- 2 years of professional nursing experience
Preferred Qualifications:
- Bachelor's Degree in Nursing
- 2 years of case management experience in a clinic or outpatient setting
