Lead Physical Therapist

At NYC Health + Hospitals/Community Care, we are seeking a highly skilled and dedicated Lead Physical Therapist to join our team. As a key member of our staff, you will be responsible for providing physical therapy services to patients in their homes, while working closely with our interdisciplinary care team.
About the RoleThis is a full-time opportunity that requires a strong passion for delivering exceptional patient care. You will be responsible for assessing patients' functional levels, developing and implementing individualized plans of care, and providing education and training to patients and caregivers on safe and effective use of assistive devices.
The ideal candidate will have a minimum of four years of professional experience in administering physical therapy under medical supervision, with at least two years in a supervisory capacity. You will also possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
We offer a competitive salary of $93,000 per year, based on your qualifications and experience. In addition, you will receive comprehensive benefits, including health insurance, paid time off, and retirement savings plan.
Responsibilities- Assess patients' functional levels and develop individualized plans of care
- Provide physical therapy services in patients' homes, including exercises and activities to relieve pain, restore function, and promote independence
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings
- Train patients and caregivers on safe and effective use of assistive devices
- Conduct regular reviews of patients' progress and make adjustments to plans as needed
- Baccalaureate Degree in Physical Therapy or related field from an accredited institution
- New York State license to practice Physiotherapy
- Four years of professional experience in administering physical therapy under medical supervision, with at least two years in a supervisory capacity
- Excellent communication and interpersonal skills
- Ability to work effectively with diverse populations
