Position Overview:

The Rehabilitation Team Leader plays a crucial role in ensuring that patient care is effectively coordinated and managed. This position involves overseeing the delivery of care and services, as well as supervising clinical staff. The leader will also assist in developing and implementing metrics to monitor and enhance process effectiveness and quality, while upholding the mission, vision, and values of VNA Health.

Key Responsibilities:

  • Oversee rehabilitation care and patient outcomes to ensure adherence to care plans and interdisciplinary collaboration.
  • Collaborate with the Director of Home Health and Clinical Management in planning, executing, and assessing educational programs for staff.
  • Work alongside management to maintain and assess the agency's budget, identifying opportunities for cost reduction.
  • Assist in the formulation and evaluation of departmental policies, procedures, and strategic objectives.
  • Facilitate discussions with management regarding individual service eligibility queries.
  • Participate in case conferences to enhance care coordination.
  • Conduct monthly record reviews, providing findings and recommendations to the Director of Home Health.
  • Support the recruitment process by screening and interviewing potential staff, providing recommendations for hiring.
  • Assist in onboarding new rehabilitation staff, ensuring all competencies are completed.
  • Contribute to performance evaluations for staff during probationary and annual reviews, focusing on clinical competencies and compliance.
  • Maintain effective communication and protocols that promote teamwork and adherence to therapy practice standards.
  • Collaborate with management to develop and assess staff development processes and professional growth initiatives.
  • Uphold professional standards and principles in all activities.
  • Engage in community outreach to promote VNA Health's role in the healthcare system.
  • Stay informed about state, federal, and local regulations related to home health service delivery.
  • Prioritize interventions that effectively address system delivery and daily care challenges.
  • Ensure adequate staffing to fulfill program responsibilities.
  • Assess community health needs and assist in developing plans to address them.
  • Actively participate in quality management initiatives.
  • Perform additional duties as assigned by the Director of Home Health.
  • The responsibilities outlined above represent a summary of the primary duties of this role; additional tasks may be assigned as necessary.

Qualifications:

  • Current licensure as an Occupational Therapist, Physical Therapist, or Speech Language Pathologist.
  • Graduate from an accredited therapy program.
  • Minimum of two years of management or supervisory experience in a healthcare setting, preferably in home care.
  • Adherence to accepted professional standards and practices.
  • Valid driver's license and access to a reliable, insured vehicle.
  • Current CPR certification.
  • Strong observation, verbal, and written communication skills.

Physical Requirements:

  • Ability to ascend one flight of stairs without assistance.
  • Capability to walk one mile, either continuously or throughout the day.
  • Ability to sit for extended periods.
  • Adequate visual and auditory acuity.
