Supplemental Health Care is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes. As a Hospice Registered Nurse, you will work with a partnering in-home hospice care provider to deliver compassionate and personalized care to individuals in need.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and other treatments as needed
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes
Requirements:
- Electronic Charting experience
- Minimum of 1 year recent relevant nursing experience
- Most contracts require some on-call
- Current Connecticut RN License
- American Heart Association BLS
- Valid U.S. Driver's License
