Responsibilities:
- Provide one-on-one care to our clients, focusing on their unique needs and goals.
- Work collaboratively with our interdisciplinary team to develop and implement personalized care plans.
- Conduct thorough assessments and evaluations to identify areas for improvement and develop strategies for optimal care.
- Administer medications and treatments as prescribed by our clients' physicians.
- Monitor and report any changes in our clients' condition to our team and their families.
Requirements:
- Current RN/LVN license to practice as issued by the State Board of Nursing.
