Job Summary
This role plays a vital part in our mission by providing compassionate and personalized care to patients in need. As a Licensed Practical Nurse, you will work closely with our interdisciplinary team to assess, plan, implement, and evaluate patient care plans, ensuring seamless transitions and optimal outcomes.
Responsibilities
- Daily Communication: Provide clear and concise communication to patients, families, and healthcare teams regarding treatment plans, medications, and equipment usage.
- Care Coordination: Collaborate with other departments to ensure timely and effective patient transfers, discharges, and post-acute care arrangements.
- Nursing Practice: Develop and maintain individualized care plans that prioritize patient-centered goals and objectives, incorporating evidence-based practices and research findings.
- Maintenance of Patient Records: Accurately document patient interactions, interventions, and progress notes in accordance with organizational standards and regulatory requirements.
Requirements
To be considered for this opportunity, candidates must possess an Associate's degree in Nursing, a current Michigan LPN license, and Basic Life Support (BLS) certification within 90 days of hire. Experience working in long-term care or post-acute rehabilitation settings is highly valued.
