Responsibilities:
- Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in our system of record.
- Administer prescribed medication, treatments, and therapies as ordered by physicians.
- Conduct patient assessments, coordinate care, and promote health education to patients and their families.
- Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
Requirements:
- Current licensure as a Licensed Practical Nurse (LPN) in the state where you will practice.
- Current CPR certification.
- Ability to provide in-home nursing care to infants, children, adolescents, or adult patients.
