Job Summary

Advocate Aurora Health is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will work with patients to develop and implement individualized treatment plans to improve their physical, perceptual, cognitive, social, and vocational abilities.

Key Responsibilities
  • Evaluate patients' physical, perceptual, cognitive, social, and vocational abilities and develop individualized treatment plans.
  • Implement treatment plans using various techniques, including assistive devices and splints, to enhance patients' functional independence.
  • Monitor and evaluate patient outcomes, adjusting treatment plans as needed, and provide status updates to physicians.
  • Plan for and complete patient discharge, including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
  • Complete all required documentation, including patient progress notes, team conference reports, discharge summaries, and records patient care and progress.
Requirements
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months.
  • Occupational Therapist license issued by the state of Wisconsin.
  • Bachelor's Degree in Occupational Therapy.
