As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.
Key Responsibilities:- Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough health assessments, develop and implement individualized care plans, and coordinate patient care with other healthcare professionals as needed.
- Order and interpret diagnostic tests, prescribe medications, and perform other medical procedures as authorized by law and hospital policy.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
- Stay current with best practices, evidence-based guidelines, and regulatory requirements to provide high-quality patient care.
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited program.
- Current licensure as a Nurse Practitioner in the state of California.
- Board certification in Family Practice or Primary Care.
- Minimum 2 years of experience as a Family Practice/Primary Care Nurse Practitioner.
- Excellent communication, interpersonal, and clinical skills.
