Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Benchmark, providing high-quality nursing care to infants, children, and young adults in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing care to clients as defined in the nurse practice act and under the direction of a Registered Nurse (RN).
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Monitor and report changes in client condition to the RN Clinical Care Manager and physician, as directed.
  • Collaborate with the healthcare team to ensure comprehensive care and support for clients and their families.
Requirements
  • Valid Indiana Nursing License
  • Valid driver's license
  • Ability to travel to nearby clients in need of assistance
