We are seeking a compassionate and skilled Home Health Aide to join our team at Lake Norman Home Health, a part of LHC Group. As a Home Health Aide, you will play a vital role in providing personalized care and support to patients in their homes, helping them maintain their physical and emotional well-being.

Key Responsibilities:

  • Provide patients with in-home personal care and designated health-related services, following the written plan of care and agency policies.
  • Assist patients with activities of daily living (ADLs), such as mobility, transfer, walking, grooming, bathing, dressing, eating, or toileting.
  • Perform incidental household services essential to the patient's health care at home, as assigned.
  • Complete clinical notes for each visit within required timeframes and incorporate them into the patient's record.

Requirements:

  • Successful completion of a competency evaluation.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
