Key Responsibilities- Assess patient needs and provide guidance over the phone
- Coordinate care with the care team, facility, and patient caregiver
- Schedule home visits as needed
- Use judgment, knowledge, and experience to ensure patients receive proper and timely care
- Graduate of an accredited nursing school (BSN preferred)
- Current state license as a registered nurse
- Minimum three years of experience as a registered nurse in an appropriate clinical care setting, home health, or hospice environment (preferred)
- Two years of hospice experience required

- Ability to sit, stand, bend, move intermittently, lift at least 25 lbs, and bear the weight of an average adult effectively
