Job Summary

We are seeking a skilled Registered Nurse to join our Home Care team. As a Registered Nurse, you will provide high-quality patient care in the comfort of patients' homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities
  • Assess patients' needs and develop comprehensive care plans
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient progress and outcomes
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Current RN licensure in North Carolina or compact state
  • 2+ years of experience in home health, hospice, or infusion care
  • Valid driver's license and reliable transportation
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
