Job Description:

As an RN Case Manager, you will be responsible for delivering comprehensive end-of-life care to our patients and their families. You will work closely with our interdisciplinary team to develop and implement personalized plans of care that address the physical, emotional, and spiritual needs of our patients.

Key Responsibilities:

  • Makes initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services
  • Identifies patient's physical, psychosocial, and environmental needs and reassesses regularly
  • Initiates and coordinates the plan of care while maintaining accurate clinical notes
  • Collaborates with patients, families, physicians, and other healthcare professionals to provide comprehensive care

Requirements:

  • Current RN licensure in the state of practice
  • One year of clinical experience
  • Current CPR certification
  • Valid driver's license and reliable vehicle or access to public transportation

Salary Range: $50,000 - $55,000 (dependent on experience)
