Job Summary

We are seeking a skilled Home Health Occupational Therapist Specialist to join our team at Advocate Aurora Health in the Western Suburbs. This role involves working closely with patients and families to develop and implement personalized treatment plans, promoting optimal health outcomes and quality of life.

  • As a Home Health Occupational Therapist Specialist, you will work closely with patients in their homes, providing one-on-one therapy sessions and education on self-care strategies to maintain independence and promote overall well-being.

Key Responsibilities

Patient Care and Assessment

Conduct comprehensive initial assessments and ongoing evaluations to identify patient strengths, challenges, and goals.

Develop and implement personalized therapy plans that address physical, emotional, and cognitive needs, promoting optimal health outcomes and quality of life.

Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication among healthcare professionals.

Treatment Planning and Implementation

Create and update treatment plans based on patient progress, adjusting interventions as needed to ensure optimal results.

Implement evidence-based therapies, utilizing standardized protocols and best practices to ensure high-quality care.

Monitor and document patient progress, making adjustments to treatment plans as necessary to achieve desired outcomes.Care Coordination and Communication

Communicate effectively with patients, families, and healthcare teams to ensure timely and coordinated care.

Participate in interdisciplinary case conferences and meetings to discuss patient progress, share expertise, and collaborate on care planning.

Maintain accurate and detailed records of patient interactions, treatment plans, and progress, adhering to regulatory requirements and organizational standards.Professional Development and Growth

Pursue ongoing education and training to enhance skills and knowledge, staying current with industry developments and best practices.

Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.

Engage in peer-to-peer learning and mentorship opportunities, sharing expertise and experiences with colleagues to foster a culture of collaboration and growth.

Requirements

  • Licensure: Current license as an Occupational Therapist (OT) in the State of Illinois or eligibility for licensure.
  • Certification: Basic Life Support (BLS) certification issued by the American Heart Association (AHA).
  • Education: Master's degree in Occupational Therapy from an accredited program.
  • Experience: 1 year of clinical experience in home health or a related setting.

Compensation and Benefits

The salary range for this position is $74,672 - $112,112 per annum, based on qualifications, experience, and location. In addition to competitive pay, we offer a comprehensive benefits package, including sign-on bonus eligibility, tuition assistance, mileage reimbursement, and more.
