We are seeking a skilled and compassionate Home Care RN to join our team at the Good Samaritan Society. As a Home Care RN, you will provide high-quality nursing care to our residents in a variety of settings, including their homes and our facilities.
Responsibilities- Provide comprehensive nursing care to residents, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that meet the unique needs of each resident.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Conduct regular assessments and evaluations to ensure the highest level of care and quality of life for residents.
- Provide education and support to residents and their families on health-related topics and disease management.
- Current RN licensure in the state of practice.
- Minimum of 3 months of experience in long-term care, with a preference for 6 months or more.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to work flexible hours, including weekends and evenings.
