We are seeking a dedicated Healthcare Coordinator-Nursing Care Manager to join our team at Childrens Home of Jefferson. As a key member of our Foster Care Department, you will play a vital role in ensuring the highest quality healthcare services for youth in our Foster Care Program.

Job Summary:

The Healthcare Coordinator-Nursing Care Manager is responsible for collaborating with our Foster Care Department and Community Clinic of Jefferson County to provide comprehensive, preventive, and emergency medical/dental/behavioral health care services to our youth. This role requires strong organizational skills, excellent people skills, and the ability to work cooperatively with others.

Key Responsibilities:

  • Completes 24-hour initial medical screens for new admissions, documents findings in our electronic medical record, and shares recommendations with relevant parties
  • Completes required HIV risk assessments/screens and provides outcomes to assigned case planners
  • Maintains documentation of each youth's medical history and treatment in our electronic medical record
  • Provides education/counseling to youth, foster parents, and birth parents on medical procedures, medication, and disease/injury management
  • Schedules necessary preventive appointments and assists families in establishing new providers
  • Maintains contact with community-based providers to ensure needs are met
  • Assists with referrals for specialty healthcare and follows up on recommendations

Requirements:

  • New York State Department of Education Licensed Registered Nurse (RN)
  • Strong organizational abilities and office skills
  • Excellent people skills and ability to work cooperatively with others
  • Ability to work with diverse populations
  • Effective and excellent oral and written communication skills
  • Demonstrated self-direction, tact, diplomacy, and ability to be clear, courteous, and professional
  • Able to react to change productively and handle other duties as assigned

Compensation and Benefits:

We offer a competitive salary range of $65,000 - $85,000 annually, depending on experience, plus a comprehensive benefits package including health insurance, retirement plan, and paid time off. A valid NYS driver's license is required.

Work Environment:

This is a full-time position that requires occasional travel to recruitment events, local schools, conferences, and meetings, as well as moderate travel within the community for off-site/in-home visits. The ideal candidate will have a strong ability to work independently and collaboratively as part of a team.
