Job SummaryWe are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Hospice Nurse - PRN. As a Hospice Nurse, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
- Collaborate with the care team to ensure continuity of patient care and make referrals to other hospice care disciplines as needed.
- Communicate patient progress to the clinical manager, physician, and care team, and document patient care progress records accurately and timely.
- Educate families on patient condition, needs, and care, and provide emotional support and guidance as needed.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- One year of nursing experience preferred, or consideration for the RN new graduate program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
