Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team of dedicated healthcare professionals. As a Case Manager, you will play a critical role in providing exceptional patient care and support to individuals in need.

Key Responsibilities
  • Meet with patients at various healthcare settings, including doctors' offices, physical therapy, and hospitals in downtown Philadelphia.
  • Develop and implement personalized care plans to meet the unique needs of each patient.
  • Coordinate with patients and healthcare providers via email, phone, and in-person meetings to ensure seamless care transitions.
  • Document patient interactions and progress in a timely and accurate manner.
  • Collaborate with a team of healthcare professionals to ensure comprehensive patient care.
Requirements
  • 2+ years of experience in a healthcare setting, preferably as a Registered Nurse (RN).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office and electronic medical records (EMRs).
  • Comfortable navigating downtown Philadelphia and walking to appointments.
