As a Psychiatric Rehabilitation Specialist at Trinity Health Corporation, you will play a vital role in providing individual and group interventions in our facilities and in the community. Our team is dedicated to delivering collaborative, person-directed, and individualized services that empower individuals to reach their full potential.

Key Responsibilities
  • Conduct individual assessments to identify strengths and areas for improvement
  • Develop and implement personalized goal plans to promote progress and independence
  • Document daily and monthly progress to track client outcomes
  • Design and teach class curriculum to promote skill-building and education
  • Maintain accurate and timely outcome reporting to inform service delivery
Requirements
  • Bachelor's degree in human services with 2 years' experience in mental health direct service, 1 year of which must be work experience in PsyR service
  • CPRP certification or must be attained within two years of date of hire certification
  • Valid Driver's License
  • Ability to drive an organization vehicle
  • Pre-Employment Drug Screen
  • Access to Transportation
