About the Role

At Elara Caring, we are dedicated to providing high-quality care to our patients in the comfort of their own homes. As a Certified Occupational Therapist Assistant, you will play a vital role in our mission to deliver unparalleled care to our patients.

Key Responsibilities
  • Implement rehabilitation plans in accordance with the Occupational Therapist's findings and ensure that all activities align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
  • Make routine visits to patients and re-evaluate their therapy needs during each visit, communicating significant findings, problems, changes in condition or environment to the appropriate supervisor, Occupational Therapist, the physician, and other team members involved with patient care.
  • Verify the Plan of Care prior to each visit and provide care according to physician's orders, assessment data, established standards and guidelines.
  • Counsel and instruct patients and/or caregivers in meeting therapy and related needs, and in the therapy home exercise program, providing requests for modification and clarification.
  • Promote compliance with all fiscal intermediary and/or other third-party payers, through education, coaching, and other assistance.
  • Identify and report potential payment/coverage problems to the appropriate Supervisor.
  • Maintain current knowledge of, and ensure compliance with all federal, state, and local regulations along with current Medicare, Medicaid, and third-party reimbursement regulations and changes.
  • Assure the continuity of quality patient care is delivered.
  • Maintain communication with clinical staff providing updates and changes in schedules, patient data or needs, physician orders, or special instructions.
  • Assure clinical notes are entered timely and indicates continuing communication and coordination of services with physician, other staff, and disciplines.
  • Implement all available actions to prevent avoidable hospitalizations and ER visits.
  • Promote Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Requirements
  • 1+ year of experience as a Certified Occupational Therapist Assistant in a clinical care setting.
  • Current State Certified Occupational Therapist Assistant (COTA) certification.
  • Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association.
  • Ability and willingness to travel to patients within branch coverage area.
  • Must have dependable vehicle, a valid driver's license, and current auto insurance in accordance with the laws of the state.
  • Meets all applicable agency policies and procedures related to health screening and required testing.
