Qualifications:

  • Current registration with the Illinois Department of Professional Regulations is required.
  • Experience in managing patients with chronic diseases is mandatory.
  • A minimum of 5 years in a healthcare environment, preferably in primary care, is expected.
  • Experience in mobilizing community resources and navigating the healthcare system is beneficial.
Job Responsibilities:
  • Facilitates team-based care within the Accountable Care Organization (ACO), collaborating with hospitals, physicians, and other healthcare providers.
  • Oversees care plans to ensure high-quality and efficient patient care.
  • Promotes a shared goal model across various settings to achieve patient and family-centered coordinated care.
  • Acts as a vital link in managing transitions of care.
Work Schedule:
  • Full-time position.
  • Monday to Friday.
  • Working hours are from 8:00 AM to 4:30 PM.
Compensation:

Starting pay is $26.00 per hour.
