- Current registration with the Illinois Department of Professional Regulations is required.
- Experience in managing patients with chronic diseases is mandatory.
- A minimum of 5 years in a healthcare environment, preferably in primary care, is expected.
- Experience in mobilizing community resources and navigating the healthcare system is beneficial.
- Facilitates team-based care within the Accountable Care Organization (ACO), collaborating with hospitals, physicians, and other healthcare providers.
- Oversees care plans to ensure high-quality and efficient patient care.
- Promotes a shared goal model across various settings to achieve patient and family-centered coordinated care.
- Acts as a vital link in managing transitions of care.
- Full-time position.
- Monday to Friday.
- Working hours are from 8:00 AM to 4:30 PM.
Starting pay is $26.00 per hour.
