We are seeking a highly motivated and experienced Registered Nurse Case Manager to join our team. As a Home Health RN, you will have the opportunity to work with patients in their own homes, providing comprehensive nursing services and direct patient care. Our team is dedicated to delivering exceptional patient care and making a difference in the lives of our patients and their families.

Responsibilities:

  • Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services
  • Work collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Provide education and support to patients and families on disease management, medication adherence, and self-care
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings

Requirements:

  • Valid Nursing License in the State of New Hampshire
  • Nursing experience required
  • Current CPR Certification required
  • Reliable transportation
