Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing exceptional care to patients with end-stage renal disease (ESRD) in an outpatient dialysis setting.

Key Responsibilities
  • Provide direct patient care to assigned patients, adhering to physician orders, state nurse practice laws, and company policies and procedures.
  • Administer intravenous medication as prescribed by physicians and in accordance with state nurse practice laws.
  • Assist in the identification, evaluation, and training of candidates for the home dialysis program.
  • Accurately document patient information, including training and treatment records, in the individual patient record and Medical Information System.
  • Conduct ongoing monitoring, assessment, and intervention for home patients, including home visits, clinical appointments, and lab studies.
  • Respond to problems or emergencies, ensuring correct actions are taken to achieve resolution.
  • Ensure compliance with patient supply procedures, documentation, and maintenance of durable medical equipment.
  • Perform monthly chart audits and report results to RN/Coordinator or Manager.
  • Monitor Peritonitis Rate and PET test results, reporting outcomes to attending physicians, Quality Assurance Committee, and Home Therapy RN/Coordinator or Manager.
Requirements
  • Graduate of an accredited School of Practical Nursing (LPN, LVN) with current state licensure.
  • One year medical-surgical nursing experience preferred, with hemodialysis experience a plus.
  • Successfully complete a training course in the theory and practice of hemodialysis.
  • CPR certification required.
  • Good communication skills, both verbal and written.
