Position Overview:
The Pharmaceutical Care Specialist plays a crucial role in enhancing patient outcomes within inpatient medical and surgical units, as well as in emergency and critical care settings. This position emphasizes the importance of effective medication management and appropriate utilization for patient safety and health improvement.

Key Responsibilities:
  • Conduct direct patient care assessments to ensure the suitability of medication therapies.
  • Provide consultation and education to pharmacy and nursing personnel regarding optimal medication practices.
  • Identify, advocate for, and implement safe medication protocols.
  • Ensure compliance with established medication-related policies and procedures.
  • Document and analyze clinical interventions and departmental performance metrics.
  • Oversee the medication reconciliation process to enhance patient safety.
  • Develop the clinical competencies of pharmacy staff and support their performance in clinical cost management tasks.
  • Mentor pharmacy students and residents as applicable.
  • Communicate and present clinical findings and departmental metrics at various committee meetings.
  • Fulfill additional responsibilities as assigned by departmental leadership.

Qualifications:
Required:
  • Bachelor's or Doctorate degree in Pharmacy.
  • Active pharmacy licensure.
  • A minimum of one year of pharmacy experience, preferably in a hospital setting, with residency or equivalent experience.
Preferred:
  • Specialized training or experience in antibiotic stewardship, internal medicine, or critical care.
  • Prior experience in a hospital environment.

Work Schedule:
  • Full-Time Position.
  • Day Shift.
  • 40 hours per week.
