POSITION SUMMARY:


The Nurse Practitioner is accountable for overseeing a designated unit or area within the facility, which includes patient evaluation (under the guidance of the Director of Nursing/Assistant Director of Nursing), medication administration, treatment documentation, and supervision of Certified Nursing Assistants and other personnel.


PART I:

KEY RESPONSIBILITIES AND STANDARDS

  • Utilizes dual identifiers to verify patient identity prior to administering medications and treatments.
  • Employs SBAR (situation, background, assessment, recommendation) as the preferred communication method when engaging with healthcare team members.
  • Encourages patient involvement in their own care whenever feasible.
  • Receives comprehensive reports from outgoing nursing staff to ensure effective communication and necessary follow-up on patient care.
  • Identifies and competently responds to changes in patient conditions, documenting appropriately.
  • Initiates and reviews the Plan of Care upon admission, making updates and adjustments as required.
  • Collaborates with all multidisciplinary teams, both internal and external, to ensure a seamless continuum of care.
  • Adheres to Standard Precautions and infection control protocols.
  • Demonstrates compliance with patient rights.
  • Refers to Nursing/Administration/IV/Emergency Management Policies and Procedures to maintain standards of care.
  • Accurately transcribes physician orders onto the Medication Administration Record (MAR) and Treatment Administration Record (TAR).
  • Facilitates timely delivery and administration of medications by communicating with pharmacy providers.
  • Maintains a safe environment for patients and reports any unsafe conditions for resolution.
  • Follows the established chain of command.
  • Practices proper hand hygiene techniques.
  • Administers medications in accordance with established policies and procedures.
  • Informs responsible parties of any changes in patient conditions.
  • Works closely with Social Workers to address patient needs.
  • Conducts checks on syringes and counts narcotics at the end of each shift.
  • Orders stock and prescription medications as necessary.
  • Facilitates patient admissions and discharges according to facility protocols.
  • Reviews reports with CNAs prior to their shift assignments.

Ensures accurate and timely completion of nursing assessments and data collection upon admission:

  • Fall Risk Assessment
  • Side Rail Screening
  • Braden Scale
  • Bowel and Bladder Assessment
  • Elopement Risk Profile
  • Pain Assessment

Contacts physician for admission orders:

  • Reconciles medications
  • Documents legibly on the Physician Order Sheet (POS)
  • Verifies and reads back telephone orders
  • Makes appropriate referrals
  • Transcribes all orders accurately.
  • Faxes back new admissions.
