Key Responsibilities
  • Conduct thorough assessments of patients' needs and develop personalized care plans to address their unique challenges.
  • Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination and communication.
  • Provide patients and their families with education, support, and resources to help them navigate the healthcare system.
  • Identify and address social determinants of health that may impact patients' well-being, such as housing, employment, and food insecurity.
  • Develop and maintain relationships with community resources and partners to ensure patients have access to necessary services and support.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.

  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in patient care.
Requirements
  • **Education:** High school diploma or equivalent required; associate's degree or higher preferred.
  • **Experience:** Minimum two years of experience in a healthcare administrative role, with intermediate to advanced proficiency in MS Office.
  • **Skills:** Excellent communication, interpersonal, and problem-solving skills; ability to work effectively in a fast-paced environment; strong analytical and organizational skills.
  • **Certifications/Licenses:** Valid driver's license and willingness to travel to multiple sites.
