Key Responsibilities:

  • Conduct thorough assessments of patients' needs and develop individualized care plans to meet those needs.
  • Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
  • Collaborate with RNs and other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes accurately and thoroughly, in accordance with Carle Health policies and procedures.
  • Participate in quarterly departmental meetings to stay informed about departmental developments and best practices.
  • Assist with coverage of extra skilled nursing visits, weekend, and holiday visits as needed.

Requirements:

  • Current LPN licensure in the state of Illinois.
  • 1+ year of experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including evenings and weekends.
