Nurse Practitioner, Adult and/or Gerontology

As a skilled Nurse Practitioner, you will deliver high-quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, and independent living communities, as well as via telehealth.


About the Role:

The PHP NP provides comprehensive primary care in a person-centered care model, diagnosing, treating, and managing acute episodic and chronic illness through wellness visits, early detection of change, and timely treatment.

  • Conduct thorough assessments to diagnose and manage chronic and acute conditions.
  • Provide health education to empower patients with informed decision-making.
  • Develop effective treatment plans and communicate them clearly to patients and other healthcare providers.
  • Order and interpret lab and diagnostic testing, adjusting treatment plans accordingly.
  • Maintain accurate documentation within our electronic medical record (EMR) software.
  • Collaborate with local and regional operations and clinical leadership to achieve strategic goals.
  • Support initiatives related to value-based care.
  • Foster an exceptional customer experience to drive high customer satisfaction and retention.
Responsibilities:

This role requires working 5 days per week, Monday-Friday, with no weekend or holiday coverage.


Qualifications and Experience:

We seek a Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate with National certification from AANP or ANCC. The ideal candidate will possess active, unrestricted state licensure and a DEA license.

  • Hold an active, unrestricted state license.
  • Possess an active DEA license.
  • Have US work authorization.
  • Demonstrate at least 2 years of clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice settings.
  • Exhibit flexibility and adaptability in a dynamic work environment.
