Nuvance Health is seeking a skilled Registered Nurse to join our team in providing high-quality patient care. As a Medical Care Specialist, you will be responsible for assessing, planning, and implementing individualized care plans for patients in our General Medicine department.
Key Responsibilities:
- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing effective care plans.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and patient satisfaction.
- Participate in quality improvement initiatives to enhance patient care and safety.
Requirements:
- Nursing diploma or graduate of an accredited school of nursing.
- Current NYS RN License.
- BLS certification required; ACLS and ECG/Cardiac Module training preferred.
Working Conditions:
- Significant manual skills and motor coordination required.
- Occupational risk and exposure to dirt, odor, noise, and human waste.
- Very heavy effort, exerting up to 50 lbs. force.
