Job Description: Day Habilitation Case Manager
We seek a highly skilled Day Habilitation Case Manager to join our team. In this role, you will develop and implement personalized service plans for our clients, ensuring their success and well-being. As part of our interdisciplinary team, you will collaborate closely with staff, individuals, and other stakeholders to create, implement, and track meaningful goals and objectives.

Key Responsibilities:
  • Develop and manage individualized service plans, adhering to regulatory requirements.
  • Collaborate with staff, individuals, and other stakeholders to create, implement, and track meaningful goals and objectives.
  • Prepare semi-annual reviews summarizing daily and monthly documentation provided by Day Habilitation Services Managers.
  • Define behaviorally stated and measurable objectives to achieve goals outlined in the PSS.

Requirements:
* Bachelor's degree or at least 5 years of case management experience
* Knowledge of funding sources and agencies
* Strong attention to detail and analytical skills

Compensation and Benefits
