In this pivotal role, you will be instrumental in delivering exceptional home care services tailored to the unique needs of our clients.

Key Responsibilities:

  • Develop and implement detailed care plans that reflect the preferences and requirements of clients.
  • Supervise a team of caregivers, ensuring they receive the guidance and support necessary to deliver high-quality care.
  • Regularly assess client progress and modify care plans as needed to ensure optimal health outcomes.
  • Facilitate communication among clients, their families, and healthcare professionals to keep all parties informed about the client's health status.
  • Maintain comprehensive and accurate documentation regarding client and caregiver activities.
  • Participate in an on-call rotation to provide after-hours assistance to clients and caregivers as required.
  • Adhere to all organizational policies and safety protocols.

Qualifications:

  • Current LPN license.
  • A minimum of 2 years of experience in home healthcare or a related field.
  • Excellent communication and interpersonal abilities.
  • Strong analytical skills for effective situation assessment.
  • Familiarity with Medicaid and Medicare guidelines.
  • Capability to work autonomously as well as collaboratively within a team.
  • Willingness to participate in on-call duties.
