Position Overview
The Hospice Admission Registered Nurse plays a crucial role in delivering compassionate and comprehensive nursing care to patients in their final stages of life. This position is dedicated to ensuring that all services align with the mission and values of LifeBridge Health.
Key Responsibilities
- Conduct thorough assessments of patients and their families to identify needs and develop tailored care plans.
- Collaborate with the interdisciplinary hospice team to ensure seamless continuity of care.
- Implement and evaluate care plans, adjusting as necessary to meet the evolving needs of patients.
- Maintain effective communication with patients, families, and healthcare professionals to provide education and support.
Qualifications
- Associate's Degree in Nursing from an accredited institution; Bachelor's Degree preferred.
- A minimum of two years of recent experience in medical/surgical nursing; home health experience is advantageous.
- Certification as a Hospice and Palliative Nurse (CHPN) is required within four years of hire.
- Current CPR certification as an AHA Healthcare Provider.
- Registered Nurse (RN) licensure is mandatory.
Skills and Attributes
- Strong interpersonal and communication skills, both written and verbal, to effectively educate and comfort patients and families.
- Ability to maintain accurate documentation and records.
- Commitment to professional development and continuous learning in hospice and palliative care.
