Key Responsibilities- Assess and Plan Patient Care: Utilize the nursing process to assess patient needs, develop individualized care plans, and implement evidence-based interventions to achieve optimal patient outcomes.
- Provide Direct Patient Care: Perform focused interviews to identify patient needs, assess signs and symptoms, and collect and analyze data to inform nursing diagnoses and interventions.
- Collaborate with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to coordinate patient care and achieve treatment goals.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Current LVN License: Possess a current and active Licensed Vocational Nurse (LVN) license in the state where you practice.
- Health Certificate: Hold a current Health Certificate (per facility) and maintain up-to-date immunizations and health records.
- PPD or Chest X-Ray: Have a current and valid PPD or Chest X-Ray result.
- BLS Certification: Possess a current and active BLS (Basic Life Support) certification.
- Experience: One year of prior LVN experience is preferred, but not required.
