Job Title: Case Manager I

Sharp HealthCare is seeking a skilled and compassionate Case Manager I to join our team. As a Case Manager I, you will play a vital role in ensuring the smooth transition of patients through the healthcare system, providing exceptional care and support to our patients and their families.

Job Summary:

The Case Manager I will be responsible for assessing, developing, implementing, and coordinating comprehensive plans of care for patients and their families. This will involve working closely with healthcare teams, including physicians, nurses, and social workers, to ensure that patients receive the best possible care and support.

Key Responsibilities:
  • Assess patients' needs and develop individualized care plans
  • Coordinate with healthcare teams to ensure seamless transitions of care
  • Provide education and support to patients and their families
  • Monitor patients' progress and adjust care plans as needed
  • Collaborate with community resources to ensure patients receive necessary services
Requirements:
  • 2 years of experience in case management or a related field
  • Current RN licensure in California
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Excellent communication and interpersonal skills
