Job Summary

We are seeking a highly skilled Clinical Pharmacist - Internal Medicine to join our team at TriStar Centennial Medical Center. As a key member of our decentralized, clinical pharmacy team, you will play a vital role in ensuring accurate medication management and patient safety.

Responsibilities:
  • Conduct thorough patient assessments to identify clinical interventions and provide recommendations to improve patient care.
  • Collaborate with attending physicians, medical residents, and advanced practice providers to develop and implement effective treatment plans.
  • Perform pharmacokinetic consults for various medications, including vancomycin and aminoglycosides, to ensure optimal patient outcomes.
  • Develop and implement TPN regimens and make daily adjustments as needed to ensure patient safety and efficacy.
  • Provide patient education and counseling on medication use and preparation for discharge.
  • Document clinical interventions and patient interactions effectively to ensure accurate record-keeping.
  • Identify opportunities for process improvements and implement solutions to enhance department efficiency and patient care.
  • Assist with pharmacist training and resident and student education to ensure knowledge transfer and skill development.
  • Maintain accurate knowledge of hospital policies and regulations to ensure compliance and patient safety.
  • Supervise and train pharmacy technician personnel to ensure effective workflow and patient care.
  • Stay up-to-date with automated dispensing processes and ensure accurate use by pharmacy and nursing personnel.
  • Retrieve, analyze, and provide accurate drug information to support patient care and treatment decisions.
  • Contribute to quality improvement initiatives, formulary decisions, and inventory control to ensure optimal patient care and department efficiency.
Requirements:
  • Advanced Cardiac Life Support certification must be obtained within 90 days of employment start date.
  • Basic Cardiac Life Support certification must be obtained within 90 days of employment start date.
  • Tennessee Pharmacist licensure or eligibility.
  • Bachelor's or Doctoral degree in Pharmacy.
  • Previous clinical hospital experience is strongly preferred.
  • PGY1 residency is strongly preferred.
