Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at the APW Jr/Sr High School-Based Health Center. The successful candidate will provide high-quality patient care, education, and support to students and families in a fast-paced school setting.

Key Responsibilities
  • Escort patients into examination rooms, initiate chart documentation, and maintain accurate records in compliance with local, state, and federal regulations.
  • Maintain a timely flow of patients according to the day's schedule, ensuring efficient use of time and resources.
  • Provide patient education materials, facilitate patient questions and answers, and execute medical regimens prescribed by licensed providers.
  • Complete electronic health records, including adequate charges for services provided, and perform tasks and responsibilities within the framework of case finding, health teaching, and provision of supportive and restorative care.
  • Maintain suite supplies, clean as necessary after each patient encounter, and participate in quality improvement, in-service workshops, continuing education, and peer review as requested by management and supervision.
  • Follow up on provider requested orders related to lab results and specialist referrals, and comply with all OSHA regulations, CLIA, NYSDOH, Quality & NOCHSI policies and procedures.
  • Engage in Patient Centered Medical Home initiatives, including participating as part of the Clinical Care Team, and perform other duties and responsibilities as requested by management and/or supervisor.
Requirements
  • Graduate of a certified nursing program, with a New York State license to practice practical nursing.
  • Ability to work well and with compassion in stressful situations, maintain a professional and courteous demeanor with patients and co-workers, and be keenly aware of the importance of confidentiality in all aspects of the position.
  • Salary: $16.80-$24.49 Depending on Experience
