Job Summary:

The Care Manager PRN at Christus Health works collaboratively with patients, families, physicians, and multidisciplinary teams to ensure seamless patient progression through the continuum of care. This role is responsible for developing and implementing individualized plans of care, ensuring effective and efficient patient flow, and promoting continuity of care, patient satisfaction, and safety.

Key Responsibilities:

  • Conduct thorough assessments of patients' social, emotional, and financial factors that may impact their health status, and identify available benefits and resources.
  • Collaborate with healthcare professionals to develop and monitor patient plans of care, ensuring effectiveness and appropriateness of services.
  • Coordinate and facilitate patient care progression throughout the continuum of care, promoting efficient and cost-effective care.
  • Serve as a resource, advocate, and support for patients and families, addressing treatment decisions and end-of-life issues.
  • Closely monitor patient length of stay and communicate with interdisciplinary teams to remove barriers and expedite discharge.
  • Identify and escalate local and system barriers impeding diagnostic or treatment progress, and address quality and risk issues as needed.
  • Work to resolve identified delays to discharge and collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient care delivery.
  • Assess needs for discharge planning and continuing care/resource support, making recommendations to patients and families regarding post-acute level of care needs and options.
  • Ensure appropriate communication and updates are provided to patients, families, and healthcare teams, and document as necessary to assure continuity of care.
  • Provide interventions demonstrating knowledge of and sensitivity toward cultural diversity, religious, developmental, health literacy, and educational backgrounds of the patient population.
  • Collaborate with physicians and other healthcare professionals to promote appropriate use of medical center resources and actively participate in multidisciplinary/patient care progression rounds.
  • Educate patients and families on available resources and support, helping them access needed services within the medical center and community.
  • Document in the medical record per regulatory and department guidelines, and assume responsibility for professional growth and development.
  • Exhibit excellent verbal and written communication skills, interacting with diverse populations, and demonstrate critical and analytical thinking skills.
  • Exercise sound judgment in interactions with physicians, payors, patients, and their families, and possess working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
  • Understand pre-acute and post-acute levels of care and community resources, and possess ability to work independently and exercise sound judgment in interactions with physicians, payors, patients, and their families.

Requirements:

  • One of the following education is required: Certificate, Associate, or bachelor's degree in nursing, Bachelor's or Master's degree in Social Work.
  • Experience in the clinical or acute care setting is preferred.
  • LVN/LPN, RN, LBSW, LMSW, or LCSW in the state of employment is required.
  • BLS is preferred.
